SURF is an international clinical audit that aims to simplify the recording and management of cardiovascular risk factors in routine cardiology clinics for patients with established coronary heart disease. A welcome initiative by University Medical Centre Utrecht and Trinity College Dublin in collaboration with the European Society of Cardiology.

This is the privacy statement for the SURF website of the University Medical Center Utrecht (UMC Utrecht) / Julius Center for Health Sciences and Primary Care. Of course, you can always withdraw your permission. If this information turns out to be incorrect, we will of course adjust this information for you.

If you haveentered personal details on a form on the SURF site, you can have these datadeleted by sending an email to UMC Utrecht or the SURF service team at the Julius Center.

It ispossible that you have a complaint about privacy protection at UMC Utrecht/Julius Center. For this you can email  the data protection officer.
