SUrvey of Risk Factors
Cardiovascular disease (CVD), especially Coronary Heart Disease (CHD), remains the leading course of death worldwide. Current clinical guidelines indicate that the priority of secondary prevention is to control and manage modifiable cardiovascular risk factors in daily practice. However, there is still a large gap between guideline recommendations and cardiovascular risk factor management in daily practice. SURF fills the gap.
Clinical audits are considered to be essential tools for recording and monitoring the standard of routine care and adherence to evidence-based guidelines, with the goal to improve the quality of patient care and health outcomes.
With this mission in mind the SUrvey of Risk Factors for Coronary Heart Disease (SURF CHD), a clinical audit, was designed. SURF CHD aims to assess the recording and management of cardiovascular risk factors and to evaluate guideline implementation in daily clinical practice worldwide. Its feasibility and applicability has previously been shown in SURF CHD Pilot and Phase I studies. A new phase of SURF CHD, SURF CHD II, will be launched in 2019.
SURF CHD can be easily administered and embedded in daily practice. SURF CHD is accessible via the internet. Filling in the SURF form by PC, tablet or smartphone takes only a few minutes.
